Wayne Memorial Community Health Centers (WMCHC) was recognized by the American Heart Association and American Medical Association for efforts to improve blood pressure control as part of the Target: BP (targetbp.org) initiative, a national evidence-based quality improvement program. WMCHC’s primary care offices earned a Gold level award based on data verifying that high blood pressure is controlled in 70% or more of the adult patients affected across its nine primary care sites.
